The tensor yarns allow the tissues and volumes of the face to be repositioned without scalpel or scarring. It is a cosmetic medical technique that can be described as a facelift without surgery.

The principle of the intervention consists in setting up a system of threads in the subcutaneous space in order to create a tension of the facial tissues.
The tensor yarns consist of a resorbable L-polylactic acid yarn on which small resorbable cones have also been placed with a bidirectional orientation. This orientation allows the tissues to be suspended after the threads have been placed under the skin.
L-polylactic acid is a material already widely used in the manufacture of sutures in cosmetic surgery as well as in classical surgery (cardiology, orthopaedics, gynaecology…) and benefits from a decline of several years demonstrating its safety. There is a real ability of L-polylactic acid to induce collagen production in the skin during the resorption process of the yarn and cones.

In general, the placement of the tensor yarns generally requires only a local anaesthesia obtained by injecting an anaesthetic product at the beginning of the session just at the very localized level of the puncture points.
The session lasts on average between 15 and 45 minutes depending on the number of areas to be treated, the extent of the improvements to be made and the possible need for anesthesia.
The number of tensor yarns to be installed varies according to many parameters, which the practitioner will evaluate as precisely as possible in advance.
Most often, two yarns per side of the face are used to treat cheekbones, nasolabial folds and jowls.
The aim of the intervention is to treat the sagging of the different areas of the face and to reposition the volumes with a natural result.
The areas generally treated are:
• the cheekbone
• nasolabial folds
• the cheeks
• jowls
• the corner of the lips
• the oval of the face
Other areas of the face may also be treated by the tensor yarns.

The application of tensor yarns can be combined with other medical and surgical techniques for rejuvenation such as laser peeling, dermabrasion, blepharoplasty or injections of botulinum toxin and hyaluronic acid that complement the treatment more naturally.
The treatment is suitable for all skin types but is not recommended in cases of allergy or sensitivity to biomaterials.
It is recommended in the first few hours after the act to remain calm, rest and refrain from any violent effort. It is also advisable to avoid as much as possible mobilizing your face (avoid facial expressions, do not talk too much, or laugh).
For 3-5 days, it is best to avoid alcohol, aspirin or anti-inflammatory drugs. Exposure to the sun and sauna are also prohibited.
The immediate consequences may be marked by the appearance of oedema (swelling) and skin redness, the extent of which varies according to the patient but generally remains moderate.
The return to social life takes 4 to 10 days.
During the first few days, at the injection sites, a certain sensitivity of the skin, itching, pallor, hyperpigmentation or small occasional bruises can be observed.
The final result of the operation can be assessed after a few days, once the yarn has settled in well, the edema has disappeared and the tissues have regained their flexibility. In this respect, the result obtained immediately after treatment does not reflect the final result.
It should be noted that in the first few days, there are often some temporary irregularities due to the inhomogeneous distribution of edema and skin, or an apparent hypercorrection (in relief) that will gradually disappear.
The treated disgraces are clearly attenuated by the yarns but do not completely disappear. Indeed, some areas are particularly difficult to treat and there are deep and old wrinkles that can never be completely erased.
That is, some areas of the face respond better than others to the treatment.
